About the Job
As a part of the Violence Intervention Program, this role provides intensive case management services to youth and families who have been impacted by community violence. This role also facilitates psycho-educational groups, develops community-based partnerships, and participates in other program activities as appropriate.

What you will do

  • Provide Case Management Services
    • Conduct consent processes and client intakes to identify service goals & case management
    • Provide case management services to youth and families including connection to community resources, medical follow-ups, substance use or mental health treatment, school transitions, housing, safety planning, recreation, legal services / support, job training, parent education and support
    • Implement a trauma-informed approach in connecting with youth and caregivers
    • Adhere to medical-legal reporting requirements
    • Complete documentation and data entry including intake and encounter / consent forms including applicable confidentiality guidelines
    • Collaborate with health and human service professionals and community agencies to ensure compliance and coordination of treatment to clients and families
    • Coordinate/provide client transportation to attend services
    • Adjust and redefine goals for clients as needed throughout involvement in the program
    • Independently manages a case load of clients and organizes workload to prioritize client care needs
    • Complete documentation in appropriate databases, including intake and consent forms, encounter forms, data, etc. in a timely manner that is legible, accurate, and organized according to department, hospital and legal standards
  • Facilitate Psycho-educational Groups
    • Support Peer Mentor in facilitating group

      * Learn BRAVE curriculum and support training of Peer Mentors and additional staff/students

      * Support with implementation of group including; invitations, reminders to clients, coordinating transportation needs, ordering supplied, documentation, etc.
  • Provides leadership in identifying barriers to effective health and human service delivery and advocates to reduce barriers for clients and families.
    • Advocate for social justice to best support youth and families

      Maintain relationships with community partners and present programs to internal and external audiences
  • Participate in Related Program Activities
    • Stay current on local activities to leverage program activities

      Participate in regular team meetings and case reviews

      Identify barriers and contribute to initiatives to improve service delivery

      * Work with supervisor to refine program materials and procedures as needed

      * Participate in research projects associated with the social programs

      * Recognize professional strengths and areas for growth and be accountable for development and enhancement of professional practice skills

      * Other duties as assigned


Licenses and Certifications

  • Driver's License - Department of Motor Vehicles - upon hire - Required
  • Licensed Social Worker (LSW) (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Preferred


Education

  • Bachelor's Degree Required
  • Master's Degree Preferred


Experience

  • At least three (3) years social work related experience Required
  • At least three (3) years healthcare related environment Preferred or
  • working with children and diverse populations Preferred


Knowledge, Skills and Abilities

  • Ability to operate a company vehicle and/or be insured on a CHOP policy (Required proficiency)
  • Basic proficiency with data management systems (Required proficiency)
  • Basic knowledge of trauma-informed care (Required proficiency)
  • Excellent verbal and written communications skills (Required proficiency)
  • Excellent critical thinking / problem-solving skills (Required proficiency)
  • Excellent interpersonal skills (Required proficiency)
  • Ability to maintain confidentiality and professionalism (Required proficiency)
  • Ability to collaborate with stakeholders at all levels (Required proficiency)
