Full Job Description
Thrive Skilled Pediatric Care seeks a passionate communicator to join our dynamic team as aPatient Care Coordinator/SchedulingCoordinator for our therapy services (physical therapy (PT), occupational therapy (OT)and speech language pathology (SLP)). This essential role communicates daily, primarily by phone, with our patient families, therapists, and team members to ensure the needs of the medically fragile children in our care are being met, that families' concerns are addressed, and ourvisits are staffed with our outstanding therapists.

Responsibilities include, but are not limited to:
*Communicating with patient families, therapists, and location team members to discuss needs and/or concerns.
*Building a rapport with patient families so they feel supported.
*Managing all aspects of the scheduling process for all patients and field team members.
*Maintaining accurate schedules and notes in our scheduling system.
*Callingtherapists daily to discuss open scheduling needs, including filling last-minute call-outs.
*Timely and direct communication with families regarding schedule changes.
Hiring Requirements:
High school diploma or equivalent required, bachelor's degree preferred.
Minimum of 2 years customer/client service experience in fast-paced environment. Home care or health care experience desired.
Prior experience in therapy scheduling preferred.
Excellent oral/written communication and interpersonal skills.
Experience in a sales environment helpful.
Experience communicating with customers/clients via telephone.
Demonstrated success in establishing relationships with a broad range of people.
Ability to process and manage confidential information.
Requires proficiency in MS Office (Word, Excel, Outlook, SharePoint).
