Hourly Pay Range (Minimum - Midpoint - Maximum):

$70.010 - $90.340 - $110.660


The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant's years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.



What You Will Do
The Program Coordinator for Patient Safety-System is responsible for leading the implementation of data-driven, evidence-based patient safety projects, using a validated methodology to achieve improved outcomes that reduce patient harm and effectively manage risk. Responsible for risk identification, prevention and mitigation of patient safety events through the application of expertise in safety and reliability science. Leads patient safety initiatives. This individual has expertise in leadership and patient safety science, including principles of error management, teamwork and collaboration, just culture, performance improvement and change management.

Required Qualifications

  • Bachelor's Degree in Nursing -OR-
  • Doctor of Pharmacy (PharmD) -OR-
  • Doctor of Medicine (MD) -OR-
  • Doctor of Osteopathic Medicine (DO)
  • 5 Years Clinical experience (ICU or ED preferred)
  • Experience analyzing and interpreting patient safety-related data to recommend specific improvement strategic priorities
  • Experience interacting, in oral and written forms, with staff, leaders, physicians, patients and families.
  • California Registered Nurse (RN) - CA Board of Registered Nursing OR California Pharmacist License - CA State Board of Pharmacy OR California Physicians and Surgeons License - Medical Board of CA -REQUIRED


Preferred Qualifications

  • Experience in healthcare risk reduction/quality/safety strongly preferred
  • Experience in conducting Root Cause Analysis strongly preferred
  • Leadership experience
  • Experience in conducting Lean Six Sigma Performance Improvement work.
  • Certified Professional in Patient Safety (CPPS) - Certification Board for Professionals in Patient Safety


Other Qualification Requirements

  • Certified Professional in Patient Safety (CPPS) is required within 1 year of hire. The department is responsible for tracking and maintaining this certification.


Essential Functions

  • Communication
    Effectively communicates patient safety related topics with the ability to professionally present results in writing or verbally to front line staff, peers, and leaders, including Executives
    Utilizes full range of software tools for dissemination of data and results.
    Presentation of information is clear, concise, and tailored to the level of the user.
    Data are complete and accurate with a keen attention to detail.
  • Educator
    Provides ongoing mentorship to Entity Patient Safety Coordinators and Patient Safety Specialists on Patient Safety Science, including, but not limited to:
    * Sharp's Cause Analysis, Response and Evaluation (CARE) Program
    * Safety Event Classification
    * RCAs, ACAs and FMEAs
    * Just Culture
    * Lean Six Sigma principles (e.g., A3)
    Participates in the development of educational materials, conducts classes, and modifies content based on feedback and ongoing requirements for:
    * Patient Safety portion of NEO
    * Just and Accountable Culture Workshop and Just and Accountable Culture Coaches Course
    * RCA, ACA and FMEA classes
    * TeamSTEPPS
    * RL Solutions Leader Training
    * Other classes, as determined
    Provides mentoring on patient safety topics to groups or individuals, as needed.
    Adjusts teaching style based on participant feedback and to effectively meet learning objectives.
    Creates educational material on relevant updates related to Patient Safety tailored to the target audience.
    Participates in the creation and dissemination of pertinent Patient Safety Alerts and other relevant education to frontline staff and leaders.
  • Meeting Attendance

    The Program Coordinator-Patient Safety is a required member at system and entity level meetings related to patient safety.
    Participates in the Do No Harm Planning Committee, tasked to plan Sharp's annual Do No Harm Conference.
    Facilitates the System RL Steering Committee in collaboration with the Director of Patient Safety.
    Co-chairs the System Patient Safety Committee in collaboration with the Director of Patient Safety.
  • Process Improvement
    Leads robust performance improvement projects using DMAIC model and/or other appropriate validated PI methodologies.
    Translates customer feedback into strategic project focus areas using Six Sigma tools, and establishes key project metrics that relate to the voice of the customer and yield process insights.
    Creates and negotiates the project charter with the project sponsor and process owner, including team, scope, resources, metrics, goals, timelines, communication plan, project transition, and project closure.
    Applies basic 'lean' principles to project management (e.g. value stream mapping).
    Creates data-driven and fact-driven project documentation using project charter, spreadsheets, storyboards, phased reviews, management reviews, presentations to the appropriate teams and councils.
    Understands and applies techniques/tools for managing organizational change using change acceleration process including identifying appropriate leaders, creating a shared need, shaping a vision, mobilizing commitment, monitoring progress and anchoring the changes.
    Identifies and effectively manages human resistance to change.
    Uses effective and appropriate communication techniques for different situations to overcome organizational barriers to success.
    Selects the appropriate metrics for projects and applies measurement systems analysis to determine and ensure accurate measurement of causes and effects.
  • Professional Competency
    Certified Professional in Patient Safety (CPPS) is required within 1 years of hire. Documentation and tracking of this certification is the responsibility of the department and will be maintained by the department.
    Greenbelt Lean Six Sigma certification is required within 3 years of hire. Documentation and tracking of this certification is the responsibility of the department and will be maintained by the department.
  • Risk Management
    Leads Root Cause Analysis and Failure Mode Effect Analysis.
    Evaluates effectiveness of risk reduction strategies and identifies opportunities to strengthen processes.
    Creates patient safety tool kits and references.
    Challenges action plans to increase the rigor and robustness.
  • Program Coordination
    Collaborates with the entity Quality Departments to plan, develop, implement and evaluate patient safety program elements:
    Cause Analysis, Response and Evaluation (CARE) Program: 1) Co-leads the System Patient Safety Committee to review entity RCAs and ACAs for appropriate cause determination and robust action plans; 2) Leads, mentors or participates in RCAs and ACAs 3) Standing member on Entity PSERT committees; 4) Facilitates spread and adoption of best practices
    Reporting of Serious Safety Events: Prepares board report for System Quality Board.
    Performance Improvement: Leads and/or participates in Performance Improvement Projects related to Patient Safety
    SharpNet: Helps maintain Sharp's Patient Safety Website
    Patient Safety Evaluation System: a) Leads RL steering meeting; b) manages RL change management process
    Safety Event Classification: a) Serves as expert and mentors others; and b) Tracks case laws to help maintain inter-rater reliability and consistency over time.
  • Leadership
    Serves as a role model and resource for the science of safety and patient safety concepts, tools, and practices.
    Collaborates with and serves as a patient safety expert consultant to the medical staff, leadership team and entity quality departments, including assisting with identification of relevant best safety practices.
    Mentors Entity Program Coordinators and Patient Safety Specialists on Sharp's internal Patient Safety Program.
    Mentors Sharp employees and leaders to enhance learning from errors and defects, to promote second-order problem solving and to advance proactive team-oriented communications.
    Defines, selects, and applies techniques that support and sustain team member participation and commitment.
    Applies coaching, mentoring, and facilitation techniques to guide a team and overcome challenges.
    Works through sponsor and process owner to ensure accountability and effective follow-through of team members to achieve project goals and timeline.
    Leads RL steering meeting and manages RL change management process to promote reporting and assure appropriate data capture and reports.
    Reviews and contributes to improving key policies and processes related to risk prevention, identification, reporting, mitigation, and correction.


Knowledge, Skills, and Abilities

  • Knowledge of regulatory requirements including: TJC, CMS, CDPH, and IMQ required.
  • Operational knowledge of computer software applications, including MS Office (Excel, Visio, Word, Powerpoint), Publisher and Cerner is required.
  • Knowledge of safety and reliability science, Lean Six Sigma, Just Culture and infection prevention is preferred.
